The year is barely days away!


Can you kindly do the following?


  1. Write down your 3 SMART health goals for the year 2023 (They should be Specific, Measurable, Achievable, Relevant, and Time-Bound)
  2. Write down 3 steps for each of the goals on what you think you can do to get there
  3. Please bring it the next time we see you so can discuss how we can help you reach them!


Example: Goal #1 Eat more fruits and vegetables

Steps: *Get a fruit basket for my house

*Buy fresh fruits and put it in said basket

*Put it where it is visible

*Eat fruit once to twice a day

*Rinse and repeat


Extra credit for starting BEFORE 2023!
