1. A written statement providing the official reason for my termination from Ben Mynatt Family of Dealerships, including the effective date and any relevant supporting details.
2. Copies of all disciplinary actions taken during my employment, including but not limited to:

• Written warnings
• Verbal warnings (with any available documentation)
• Performance improvement plans
• Any other documented corrective actions placed in my personnel file

Please email these materials to ricoglover@marketingdisruptors.com. If for any reason you are unable to provide digital copies, I would appreciate being informed of the process to obtain them in hard copy.

I respectfully request these records be provided within ten (10) business days of this email. If additional information or authorization is needed to fulfill this request, please let me know.
