Does Dr. Malik accept my insurance?

In order to deliver excellent care without the restrictions of commercial insurance companies and spend more time with each patient Dr. Malik will be out of network with all insurance plans.


How will I know how much an appointment will cost me?

To determine your insurance coverage for out-of-network healthcare services, it is advisable to review your insurance policy or directly contact your insurance provider. Your policy will typically provide information on the percentage of the out-of-network cost that your insurance will cover and your out-of-network maximum, deductible and copayment. It is also recommended that you confirm if your insurance provider requires pre-authorization or a referral for out-of-network services.


Prior to your appointment, our office will provide you with a detailed breakdown of the payment due for the services rendered, which is payable on the day of the appointment. Following the appointment, you will receive a superbill, which can be submitted to your insurance provider for reimbursement of covered out-of-network expenses.


Will Dr. Malik have in-person visits?

Yes, the address, phone number, and availability for appointments will be shared with those on the waitlist as soon as it is available
