Our OCPhA board and committees are asking for your feedback on how OCPhA can better serve our members and shape future programming, including possible events, incentives, support, engagement opportunities and more!
This short survey will only take <5 minutes of your time. As a thank you, you'll be entered into a raffle for a chance to win 1 of 8 prizes:

This survey is live from now until September 30th, 2025. This raffle draw and winners will be announced at the October OCPhA virtual meeting which will be on Thursday, 10/2/25 at 7:15pm via Zoom (bit.ly/ocpharmacist). One entry allowed per member; for duplicate entries, we will go based the latest entered information in the form by the the survey deadline.
Winners must be present online during the October virtual meeting to receive their raffle prize. More raffles will come throughout the year, so keep an eye out for more opportunities in the future!
