Hey ,
This session with Susan Hastings and Kelly is just jam packed with clinical take-aways! Have a listen in on how they problem solve crouch gait and more for the gorgeous Penny. A 3-year-old with diplegic CP post SDR.
Here's a few things you'll want to check out from this session:
- Introducing NMES to a young child, electrode placement and parameters used.
- Recognizing behaviour as a sign that an activity is too challenging, even when it seems that it's within the child's abilities.
- Noting effect and setting expectations for progress

Some clinical take-aways
- Focus on dorsal muscles for extension rather than ventral muscles
- Adductors help extend knee and hip, shifting patient backward
- Gastrocnemius strength is crucial for standing (50%)
- Slow down rapid walking to improve gait pattern
- Keep feet no wider than shoulder-width apart
- Use larger electrodes to cover more of the muscle bulk
- Consider combining E-stim with bracing for vertical tibia alignment
- Strengthening comes as a result of motor learning, not vice versa

I loved watching Susan and Kelly work in real time to introduce eStim to Penny and I especially loved Kelly's reflection on this session, it just highlights the real time learning that comes from real clinicians doing real clinical work with real kids in real time ♥️
I learned a lot of new information about crouch gait that I was unable to learn even after lots of time spent researching on the internet. Such as that the adductors are important to activate w/ e-stim due to their underappreciated action of extending the hip. I also had never considered to activate the plantarflexors since we were so lacking in dorsiflexion but this also makes sense to do this to further assist in activating knee extension.
