ASKP3 Statements, Concerns and Recommendations regarding At-Home Ketamine

The American Society of Ketamine Physicians, Psychotherapists & Practitioners (ASKP3) has released guidelines addressing the growing interest and controversy around at-home ketamine use. At Innerbloom, we do not advocate for at-home ketamine use due to concerns about safety and lower efficacy, emphasizing that ketamine is best administered in medically supervised settings. The guidelines also discuss the pharmacokinetics of oral ketamine, noting its lower bioavailability and variable effects compared to intravenous administration.

Ketamine vs. Electroconvulsive Therapy (ECT) for Treatment-Resistant Depression (TRD):

For patients with treatment-resistant depression (TRD), there are several options available, including traditional treatments like electroconvulsive therapy (ECT) and newer approaches such as IV ketamine. A recent study highlights the differences between these two treatment methods.


▪️Response Rates: Ketamine provides greater symptom reduction than ECT for moderately severe to severe depression. For very severe depression, both treatments yield similar outcomes.


▪️Setting: Ketamine is particularly beneficial for outpatients. In contrast, ECT may be more suitable for inpatients, especially those with very severe depression.


▪️Side Effects and Relapse: Ketamine may cause short-lived side effects such as nausea and dizziness. In contrast, ECT can be associated with persistent side effects like decreased memory recall, and it has high relapse rates, with most patients relapsing by the 6-month follow-up.

The FDA Advisory Committee votes 'No' on MDMA: While we patiently await the final word on August 11th regarding the potential FDA approval of MDMA for the treatment of PTSD, the Numinus team provides some insights into why the "pre-approval" meeting in June went unfavorably for advocates of MDMA-assisted therapy. Concerns raised included issues with study design, underreported drug reactions, and unstandardized methods of psychotherapy.
