HELLO CFC PROSPECT GAME PARTICIPANTS

The reason for this email is to get everyone to let us know who the chaperone your son will be with in Ottawa.

The below is the Google Sheet where we need you to put this information. It is organized alphabetically by first name.

Please take a few minutes to fill this out as soon as humanly possible. We use this document in case we need to contact someone from practice or game facilities.

If you are local and will be staying at home just write in the columns ‘local’.

If not, please indicate the person your son will be staying with in Ottawa. This could be you, a relative (grandparent, aunt, uncle, etc.) or a friend. If not you, basically someone you have given permission to be your son's chaperone while in Ottawa.

Please add the name, mobile number, and relationship of the Ottawa Chaperone.

Please note that CFC does not take responsibility for chaperoning players outside of the practice or game facilities.
